SUMMARY:DIABETES WA-"LET'S PREVENT" PROGRAM
DESCRIPTION:The “Let’s Prevent” program is run by Diabetes Western Australia. The program is designed to help prevent or reduce an individual’s risk in developing type 2 diabetes\, strokes or cardiovascular disease. It is a health program that provides people with practical information and skills that will enable them to make simple\, sustainable lifestyle changes. “Let’s Prevent” is an evidence-based program from the UK which has been adapted for the Australian community. It has a fantastic NPS and PAM score rating and has been proven to support behavioral change. \nOUTLINE OF THE PROGRAM \nThe “Let’s Prevent” program is structured over a 12-month period where two local health facilitators run the initial 6-hour session. A maximum number of 10 patients are able to attend and each patient can bring along a support friend or family member. \nThe 6-hour workshop addresses the following: \n\nCore nutritional principles such as\, “How food gets absorbed and the role carbohydrates and glucose play in the body”.\nHow does pre-diabetes develop into diabetes?\nTypes of fats and how they impact health\nInteraction with food models and understanding portion sizes\nBlood vessel health\nThe importance of physical activity\nSMART goal planning\nPsychosocial wellbeing\n\nAfter the initial session has completed\, patients are contacted at 8 and 16 weeks for a follow up\, support call by a health professional. After 6 months\, patients attend another 3-hour session followed by a 12-week support phone call. At 12 months the patient ends the program and are referred onto other health professionals or organizations if they require further assistance. \nSOUTH WEST REGION PILOT STUDY \nDiabetes WA has been conducting a “Let’s Prevent” pilot study in the South West/Greater Bunbury area over the last two years. The study results showed an average weight loss of 5.4 kg in participants over a 4-month period since starting the program. More than two thirds of the participants increased their physical activity and near on 60 percent consumed more vegetables in the 6 months since attending the initial session. \nWHO CAN JOIN THE PROGRAM? \n\nThe program is aimed at helping people who are at a high risk of developing Type 2 Diabetes\, CVD or strokes. If you notice your patient’s blood profile increasing overtime (HbA1c\, Fasting BG\, raised BP or cholesterol level) or if your patient has an AUSDRISK score of 12 or more or if your patient is obese\, please send them onto the “Let’s Prevent” program as soon as possible. Diabetes WA provides a “Let’s Prevent” form to all health professionals that will help facilitate easy patient activation onto our program. \n  \nPROGRAM DATES IN GERALDTON \n  \n12 February 2021 \n27 March 2021 \n22 April 2021 \n  \nCONTACT \n  \nPlease contact tanya.dale@diabeteswa.com.au if you require further information or you can visit diabeteswa.com.au/lets/prevent. \n
