This week’s topic at the Friday Morning Breakfast will be :

“ Heart failure Optimization of Health, Patient management and Education (HOPE) Study: Attempts to standardise Heart failure management in the Midwest Region of Australia”. 

This study evaluated the prevalence of heart failure admissions at GRH over one year (2021/2022) and assessed if patients were discharged with optimal therapy. It aims to bring the care provided at GRH in line with the Australian Heart Failure Clinical Guidelines and will assess the need for a dedicated Heart Failure service at GRH, by retrospective review of medical records. By assessing and quantifying the need, a targeted and sustainable solution for implementation at GRH was designed, with the aim of enabling seamless transition of heart failure patients to primary care.”
